根据以上原则我们首先分析腰椎的动作：腰椎因没有肋骨的牵制，活动范围较大，但同时其支持的系统主要靠肌肉软组织支撑，容易导致肌肉软组织劳损。根据其运动方向有三个运动轴，即前屈、后伸；左、右侧弯；左、右旋转。下面分析每个活动轴易损伤的软组织。 （1）前屈动作（即弯腰的动作，正常人的腰部前屈范围为90°）：此时腰椎的上部相对稳定，主要的弯屈动作在下腰部，以腰骶部为明显，所以该弯腰的动作易引起腰骶三角区（即腰椎、骶椎和髂嵴后部形成的夹角称腰骶三角区）所在的软组织形成劳损；后伸动作（即仰腰的动作，正常人的腰椎后伸活动范围为30°）此时下部腰椎相对稳定，主要引起弯曲为上部腰椎的动作，以腰肋三角区（腰椎与肋骨形成的夹角称腰肋三角）所在的软组织易形成劳损。 （2）左、右侧弯动作：正常人腰椎左、右侧弯活动范围为20°，腰椎在做左右侧弯时，腰部两侧的软组织被牵拉，一般侧方软组织的肌筋膜附着在腰椎的横突上，而腰椎横突以第三腰椎的横突长（也有变异的如第四腰椎横突长）。长的横突上附着的肌肉多，应力也，加之因其长与周围软组织形成摩擦的机会多，也是易劳损的部位，所以左、右侧弯动作易损伤的为第三腰椎横突。 （3）左右旋转动作：正常腰椎左、右旋转的活动范围为30°。腰椎的左右旋转动作，也就是日常生活中的翻身动作，其参与肌肉为腹内、外斜肌，腹内斜肌肌块大、肌力强，腹外斜肌的肌块小、肌力弱。在同一动作中，易劳损的肌肉自然是腹外斜肌，另外在腰椎椎体的前侧方，也有块肌肉为腰大肌，当其劳损痉挛时也会牵制腰椎的翻身即左、右旋转的动作，所以腰椎左、右旋转动作受限时应考虑腹外斜肌损伤和腰大肌损伤。另外腰骶三角区也会导致翻身困难，在临床诊疗时要加以区别、灵活掌握。
